Gather information and research similar predictive models
2. Gather Clinical Feedback
Convene a group of people across disciplines (MD, RN, Case Manager, etc.) to brainstorm potential predictors
3. Generate Data Elements
Query data sources to pull each potential predictors into a table or extract files that the statistical modeling tool can use
4. Develop a Model
Run potential predictors through a regression model to determine which elements are predictive, generating an overall risk score

Define the patient population and workflow then implement tools to operationalize the prediction model
6. Pilot, Rollout, and Use
Train care managers and others who will be using the tool
