Navigating Care Transitions – Topic Call Today

IMG_7731

It’s not just because I’m in CA and will be driving over this bridge today that we chose to share this picture for our blog. (Truth be told though, I am loving the weather – so much warmer than Minneapolis)! But seriously, what does the Golden Gate Bridge have to do with Care Transitions? Well, join us for an upcoming Topic Call with the Beryl Institute and find out!

Sometimes considered the composite late to the HCAHPS party, Care Transitions is an important and not yet tackled one across the country. Join us as we discuss how Care Transitions connects with other HCAHPS and quality metrics and some key tools and strategies to utilize for effective care transitions.

Today, 1/11 at 2 p.m. EST, I will be facilitating a Topic Call for The Beryl Institute. The topic is Care Transitions and we’ve attached some info and registration links below. We led a webinar on this topic right before the holidays which was very well received. Many in the patient experience community are clamoring for some info on this as there’s not much out there just yet. We hope you can join us!

I began my career as a social worker and later, with my Master of Health Administration (MHA), directed operations at Allina Health in Minnesota. I later directed patient experience at Allina. My background and experience give me strengths in approaching healthcare opportunities and challenges through a systems lens – with unique strengths, challenges and activation points. My experience is also informed by leadership roles leading performance improvement in patient-centered care and patient experience departments. Working in the client role with healthcare consulting firms inspired me to address some unmet needs in the industry, and to co-found DTA Healthcare Solutions. I have a Lean Six Sigma Black Belt, and find that the discipline of practice translates to healthcare work very well.

We’ve helped clients across the country accelerate toward value-based healthcare delivery.

Let us do the same for you.
